Effective January 1, 2004, the Company commenced operations in Ohio through the acquisition of the Medicaid-related assets of Family Health Plan, Inc. Effective December 1, 2004, the Company acquired FirstGuard, Inc. and FirstGuard Health Plan, Inc.
